The goods and services shown in this General Price List are those we can provide to our customers. You may choose only the items you desire. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. Unless legal or other requirements mean you must buy any items not specifically asked for, you will be charged only for those items you select. There may be extra charges for other items, such as cemetery fees and flowers. All of our prices are itemized.
Every attempt has been made to present this General Price List in a format that families will understand. The step-by-step process allows families to pick those services they desire as all fees are itemized. In addition, several Minimal Service Packages are available in Category A - Section 8 below.
In-person Arrangements to Obtain Necessary Permits and Records
Coordinating with person or person authorized to make final arrangements. - In-person consultation with a
licensed funeral director to gather information needed to obtain permits and records - Recording vital statistics -
Submitting information to State of Florida and other governmental agencies - Obituary preparation for one local
newspaper - Courier personnel and vehicle to retrieve requested documents.
Please note: This internet-based fee includes only those items listed above. This fee does not include, the coordinating of services and ceremonies, courier personnel to retrieve requested documents, or obituary preparation. Such services are itemized charges.

PREPARATION/SHELTERING
Embalming
Except in certain special cases,embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. It is our funeral home’s policy to require embalming if the body is to be present at any rites or ceremonies.

Note: In normal circumstances the casket must be closed during a graveside service our of consideration for
other families who may be visiting the grave/s of their loved one/s at the cemetery.

ID View (Private Family Viewing) (if condition of the deceased allows)
Immediate family members only (Limited to Spouse, Children, Grandchildren, Parents,Sisters/Brothers only) for
15 minutes maximum. Scheduled at the convenience of our firm between the hours of 9 a.m. – 3 p.m. –
Monday through Friday in a facility at our Manatee Avenue Funeral Home.
Reception - Facilities/Staff Services (2 hours)
Use and set-up/clean-up of our Stateroom - Attendance of one dedicated staff member in our Stateroom for a
2-hour period - Provision of coffee and hot tea, as well as plastic cups, platters, pitchers, and cutlery as
dictated by any additional refreshment provided by the client.

Scheduled between the hours of 9a.m. – 4 p.m. Monday through Friday. (Does not include the cost of a third-
party pathologist, nor licensed staff supervision.)

MINIMAL SERVICE PACKAGES
This charge includes transfer of the deceased to this funeral home - gathering information via the internet -
refrigeration - cremation fee (on-site crematory), - preparation of free death notice in one local newspaper -
cremated remains returned in a temporary cardboard container. It does not include courier personnel - ordering or
procuring death certificates.
If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the
body and can be made of materials like fiberboard or composition materials (with or without an outside
covering.)The containers we provide are heavy duty cardboard.
This charge includes transfer of the deceased to this funeral home - gathering information via in-person
conference - refrigeration - cremation fee (on-site crematory), courier personnel to retrieve and/or transport
documents - obituary preparation for one local newspaper (3 edits) - cremated remains returned in a temporary
cardboard container.
If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the
body and can be made of materials like fiberboard or composition materials (with or without an outside
covering.)The containers we provide are heavy duty cardboard.
This charge includes transfer of the deceased to this funeral home - gathering information via in-person
conference - refrigeration - casketing - and funeral coach. Immediate Burial is scheduled at the convenience of
the funeral home and cemetery and does not include family participation or services/ceremonies of any type at
the cemetery.
Forwarding of Remains to Another Funeral Home in US & Canada (without service or viewing)
This charge includes transfer of deceased to this funeral home (within 15 miles. Additional mileage fees apply
after 15 miles) - gathering information via in-person conference - embalming (required by airlines) - sheltering for a
period of 48 hours - obtaining necessary permits - transportation to Sarasota/Bradenton International Airport in
our hearse. It does not include any merchandise including a casket or shipping container, viewing, funeral rites or
ceremony or the coordination of such ceremonies. Nor does it include the charges of a common carrier.

This charge includes receiving the remains from Sarasota/Bradenton Airport, the overnight sheltering of the

crematory. It does not include any merchandise including a casket, viewing, funeral rites or ceremony.
